Excessive use of drugs creating resistance to antibiotics: report






KARACHI: An excessive and irrational drug use across the health sector in Pakistan is creating resistance to frontline antibiotics used to control infections, says an official report.
The recent report said that antimicrobial resistance resulted from irrational use of drugs and showed that a high level of resistance to ampicillin, cotrimoxazole, chloramphenicol and erythromycin had long since been established in Karachi. “The results are alarming as these are the frontline antibiotics for control of infections,” it said.
The report said the irrational drug use was rampant despite the fact that Pakistan was one of the first countries to introduce the World Health Organisation’s (WHO) essential medicines programme.
The report pointed out low checks on registration and marketing of drugs and said there were 1,100-1,200 registered molecules and about 76,000-88,000 registered products in Pakistan, which was one of the highest in the developing world.“
Unnecessary number of registrations is due to both low quality checks required for registration and reportedly high influence of pharmaceutical industry. This contributes to irrational use of drugs with availability of excessive number of originator brands at different dosages and prices,” it said.
Moreover there was ‘almost no restriction’ on health providers interacting with industry. A visit by industry representatives to general practitioners “is alarmingly often the only source of treatment information, underscoring lack of in-service training, however, information provided is questionable”.
The report quoted a study saying some 18 per cent of sales advertisements “have unjustified or misleading claims” while another study showed that only 15 per cent of claims in promotional brochures met the WHO criteria for optimal drug information.
It quoted a study conducted previously by a private consultant saying self-medication and drugs purchased over the counter from medical stores of “questionable quality” was rampant.
“Although there are no national figures on self-medication, available studies indicate that in at least 51 per cent of ailments medicines are purchased over the counter through self-prescription. There is little restriction over type of medication that can be purchased and quality of drug stores is poor,” it said.
WHO defines the essential drugs as “those that satisfy the healthcare needs of majority of population; they should therefore be available at all times in adequate amounts and in appropriate dosage forms at a price the community can afford.”
However, the recent official report said the prescriptions by non-qualified practitioners as well as self-medication by patients were common in Pakistan.
To authenticate its contention, a previous study conducted in Karachi said potent drugs such as antibiotics, psychotropic, narcotics, anti-cancer therapy and harmones were frequently misused due to inappropriate prescriptions by quacks who mostly operated in remote rural areas of Sindh.
Estimates of the Pakistan Medical Association showed that there were more than 600,000 quacks operating across the country, of whom one-third were in Sindh and of them most were in Karachi.
At present, there is no official legal mechanism, through which authorities could monitor and take action against fake doctors.
The recent study, however, held the licensed practitioners responsible as well for recommending high level of inappropriate prescriptions to their patients. “Many qualified medical practitioners frequently prescribe inappropriately even for endemic problems,” it said.
In Karachi, it said, appropriate therapy for hypertension in elderly people was given by only 35 per cent of general practitioners (GPs). There were frequent variations from the recommended tuberculosis treatment with only 50 per cent of GPs being able to prescribe TB drugs in correct doses or correct duration.
Similarly, the treatment for psychiatric and paediatric illnesses did not correlate to diagnosis in 25 per cent of cases and doses of drugs were inappropriate in 31pc prescriptions.
Moreover, GPs in 76.5pc of patient encounters also dispense drug formulations of unknown composition, commonly known as ‘mixtures’, made in their own drug dispensing corner – a practice, which is not open to monitoring.
The report also highlighted a high number of drugs per prescription and rampant use of injections. The average number of drugs prescribed per patient is three or more in Pakistan as compared to an average of two to three in low- and middle-income countries, and over 70pc of patients are prescribed antibiotics.
Across Pakistan, 2.75 drugs per prescription are reported from basic health units and rural health centres, which is close to the average of 2.79 at teaching hospitals, showing low rationality at lower tiers.
The number of drugs prescribed is higher in the private sector with an average of 4.5 medicines per prescription in urban Sindh with over-prescriptions of antimicrobials, vitamins or minerals and injections.
‘Military should be used sparingly in maintaining law and order’



Civilian oversight over the military is firmly entrenched in Indian democracy. Yet there are bouts when the ties between civil and military establishment become uneasy and volatile. Therefore, understanding civil-military ties in India remains a daunting task for the uninitiated. 

In an e-mail interaction with Zee Research Group’s Ajay Vaishnav, Ayesha Ray, who recently published ‘The Soldier and the State in India’, offers a theoretical perspective to delve into the critical issues that have emerged in Indian civil-military relations. The book uses Samuel Huntington’s ideas on military professionalism and Peter Feaver’s discussion of military expertise in the American context as the theoretical framework for addressing similar issues that have emerged in debates on Indian civil-military relations. Ray is an Assistant Professor in the Department of Political Science at King’s College, Pennsylvania. 


Q1. What motivated you to explore civil-military ties in India?

There were two driving forces that shaped my initial interest in studying Indian civil-military relations. First, I hail from a family of officers who served in the Indian military. Growing up in a military family shaped my immediate interest in examining issues of peace and war more closely. Second, while undertaking research as a graduate student both in India and America, I found the literature on Indian civil-military relations rather sparse and wanted to make a contribution that could help explain the changing relationship between India’s political leadership and the military over time. 

Q2. What factors keep Indian military subservient to the civil-political establishment even as we have seen juntas ruling in Pakistan, Bangladesh and Myanmar for long durations?

The political leadership in India exercises complete civilian control; a hallmark of most thriving democracies. The Indian leadership’s caution in giving the military too much power can be traced back to Nehru’s fears of a powerful military that could have challenged civilian supremacy. In the context of India’s independence and partition, Nehru’s distrust in the Indian military helped institutionalise a very tight form of civilian control, thereby preventing it from going the route of Pakistan, Bangladesh, and Myanmar. Therefore, the role of elite and the institutional apparatus of higher defense organisation in India have kept the military under tight civilian control. 


Q3. How Indian military is engaged in policymaking? Is there a statutory framework to engage them or factor in their viewpoint on key policy decisions?

In my book, I argue that India’s overt nuclearization, and the frequent use of the Indian military in counter-insurgency operation has generated friction in the relationship between civilians and the military, consequently giving the military an avenue to make its concerns on policy public. While this in no way implies that the military will challenge overall civilian control, it does point to various issues that have increased disagreement between the two domains giving the military a somewhat greater leverage or bargaining power in policy decisions. 

Q4. If disagreement(s) becomes irreconcilable, what are the chances of military ever attempting a coup in India? Last year a report surfaced in an Indian English daily about an attempted coup by the Army, which was rubbished by the government. 

That’s always a legitimate concern for any democracy. I think last year’s reports of an attempted coup were exaggerated in the media. Also, public statements made by the military’s top brass revealing disapproval with political directives do not necessarily imply an attempt to seize power. Despite contentious issues between the military and civilian leadership, democratic control over the armed forces in India has and will always remain tight. Our institutional apparatus of higher defense organization reflects this political and bureaucratic control over the three services. Also, the Indian military prides itself in maintaining a high degree of professionalism which should act as an internal check against any possible seizure of power. 

Q5. What is the way forward to improve civil-military ties in India?

Greater communication and transparency between the military and political leadership might help reduce civil-military friction. Also, in accordance with existing external and internal security threats, the government must follow through on its initiatives to reform institutions in higher defense organizations to maintain a robust and professional war fighting force. Moreover, the Indian military should be used sparingly in the maintenance of law and order as that moves it further away from its conventional war-fighting functions. 

Q6. Why do you suggest the military should be used sparingly in the maintenance of law and order?

If the military is routinely called in to assist civilians in the maintenance of law and order as evidenced in cases of internal insurgency and insurrection, its traditional war-fighting functions (defense preparedness against external threats) can get adversely affected in the long term. Also, “stretching” the military’s traditional functions always carries the danger of politicizing the military. Hence, in any democracy, the military should always be used cautiously. Such contentious issues are emerging in American civil-military relations too with the over-stretching of the American military in fighting ill-defined wars abroad.

Uttarakhand: 127 more bodies recovered, toll mounts to 807





Dehradun: The Centre and the Uttarakhand government on Tuesday informed the Supreme Court that the rescue and relief operations and the evacuation of those stranded across the flood-hit state will be completed in the next 72 hours. 

In its Action Taken Report (ATR) submitted to the Supreme Court, the state government said that the clearing of debris from different areas is high on its agenda. 

Responding to a query from the Supreme Court about the rescue work being completed so far, the state government said that it aims to complete the evacuation of all the stranded pilgrims in next 72 hours if weather remains favourable. 

The Action Taken Report also says that the top priority is identification and disposal of dead bodies. Efforts are being made for listing of missing people, the report added. 

The submission was made after the Supreme Court asked the Centre and Uttarakhand government to step up rescue operations to evacuate people caught in flash floods in the state. 

A bench of justices A K Patnaik and Ranjan Gogoi said the governments should consider the suggestions to be submitted by the petitioner who had moved the PIL for the rescue of people and posted the case for further hearing on June 28. 

In its ATR, the Centre said that a total of 1189 air sorties were done by the Indian Air Force while the Army has carried out 419 sorties. 

By June 23, total 96,500 people were evacuated by all agencies and free transport is being arranged for all people to reach their respective destinations and more than 4000 vehicles have been deployed to bring the stranded people to Haridwar and Dehradun, the ATR said. 

The report said that 80 per cent of telecommunication facilities have been restored and 77 satellite phones have been distributed by the Centre to various agencies. 

The state government further said there is no shortage of food items and essential supplies in the disaster affected areas and 50,000 bottles of water and 50,000 packets of biscuits and 45,000 ready to eat food packets have been made available and the government is organising relief camps for the disaster affected people at 62 different places. 

The report said, that approximately four lakh chlorine tablets have been distributed in the affected districts and another five lakh would be sent in another two days.

At least 127 more bodies were recovered from Kedarnath taking the death toll due to the natural calamity to 807 even as security forces began the process of cremating decaying corpses lying in the shrine premises.

Foggy and overcast conditions in Dehradun delayed take off by choppers at Sahasradhara helipad and Jolly Grant Airport but air rescue operations to evacuate around 9000 people have now resumed with improvement in the weather. 

With rain hampering rescue operations from Badrinath where the majority of pilgrims remain stranded, Chief Minister Vijay Bahuguna has appealed to their relatives not to run out of patience saying they are safe with enough supplies of food and and medicines made available to them. 

Assuring them that their kin stranded in Badrinath will be evacuated safely at the earliest, he asked them not to worry. 

There are about five community kitchens being run at the shrine under the supervision of the district administration, he said. 

Meanwhile, truck loads of dry Deodar wood and ghee have been despatched to Kedarnath and the effort is to begin the process of mass ritual cremation of bodies strewn over the premises after their identification, post mortem and DNA preservation formalities, sources said. 

With the bodies already beginning to decompose, the air is laden with a stench giving rise to fears of an epidemic outbreak in affected areas.

Met warns of more rains 

Meanwhile, the Met department has warned of more rains over the next 24 hours in Uttarkashi, Chamoli and Dehradun. 

"We're expecting rains across the state. Heavy rains are possible in Uttarkashi and Chamoli and rescue operations will be affected. Rains across the state are expected on Wednesday too,” a Met department official was quoted as saying..
